The NHS Health Check is for people aged 40-74 and is designed to spot early signs of dementia, heart disease, kidney disease, stroke or type 2 diabetes.
If you fall within this age group you should expect to receive a letter from your GP, or local authority, inviting you for a free health check every five years.
Oxfordshire County Council want to know if residents have had their NHS Health Check, and if you haven’t, why not.
